Should you wish to contract with NewMarkets Insurance Agency, please note the following procedure:
Mail or fax all of the following documents to the address listed to the right:
Completed
Application
Signed Producer Agreement
Proof of E&O Coverage
Completed W-9
A copy of all agency and individual licenses in states where you expect to submit business, including both the P&C and surplus lines licenses.
These documents are subject to compliance verification. If approved, you will be notified and an executed copy of the agreement will be mailed.
